Immunization Requirements for Incoming 7th Graders

Woodland Middle School wants to make certain that your child is ready for 7th grade. This is a reminder to connect with your child’s health care provider or the Health Department to ensure that your child’s immunizations are up-to-date. Spring Break and Summer break are perfect times to obtain this.

• TETANUS-DIPHTHERIA-PERTUSSIS BOOSTER (Tdap) IS REQUIRED FOR 7TH GRADE ENTRY if none have been received within the last five years. It is important to know that without appropriate documentation your child will be unable to attend classes beginning August 12, and will also be unable to pick up his or her schedule during our Back to School Orientation that will take place on Thursday, August 8th

Once your child has all of the required immunizations, please bring the completed Department of Health 680 Form to Woodland, so that we can update your child’s records immediately, and allow for a smooth transition to 7th grade.
