Woodland Middle School wants to make certain that your child is ready for 7th grade next year. This is a reminder to connect with your child’s health care provider or the Health Department to ensure that your child’s immunizations are up-to-date. Summer break is a perfect time to obtain this.

TETANUS-DIPHTHERIA-PERTUSSIS BOOSTER (Tdap) IS REQUIRED FOR 7TH GRADE ENTRY if none have been received within the last five years. It is important to know that without appropriate documentation; your child will be unable to attend classes. Once your child has all of the required immunizations, please send the completed Department of Health 680 Form to Woodland (lisa.phillips@sarasotacountyschools.net), so that we can update your child’s records.
