Meet Your Teacher: Friday August 15, 2008

12:00-2:00pm

SARASOTA COUNTY REGISTRATION INFORMATION

To enter Kindergarten your child must be five years old on or before September 1st.

WHAT TO BRING FOR REGISTRATION

A parent or Guardian must accompany a student to register at a school. Florida law requires the following;
1) Written proof of current residence address (e.g. recent utility bill, water, electric, mortgage or lease agreement.)

2) Proof of date of birth - need one of the following:

A. Certified copy of birth certificate
B. Baptismal certificate showing date of birth and place of baptism, with parent's sworn affidavit
C. Insurance policy on child which has been in force at least 2 years
D. Bible record or child's birth with parents' sworn affidavit
E. Passport or certificate of arrival in U.S.
F. School record, at least 4 years prior, showing date of birth
G. Parents' sworn affidavit accompanied by a certificate of examine from a health officer or physician verifying the child's age

3) Proof of immunization verified with HRS Form 680. This form is available from a private physician or the Health Department

4) Physical exam within the previous 12 months signed and dated by a physician
5) Proof of Custody. If a single parent is separated, divorced, or if other custodial situation exists a copy of legal residential custody document is required

You should also bring:

1) Name and address of previous school(s) attended
2) Any available information from previous school(s) such as report cards, transcripts or student participation in special programs
3) Student's Social Security Number - bring card with you (it will become student identification number)
