Cranberry Elementary School Registration Requirements  

REQUIREMENTS FOR KINDERGARTEN REGISTRATION

CHILD MUST BE 5 YEARS OLD ON OR BEFORE SEPTEMBER 1

1. Official birth certificate or passport, if born out of the USA .
2. Written proof of residence:  a signed lease, FPL bill, or water bill which includes YOUR NAME AND ADDRESS.
3. Health physical exam ***See explanation on Registration Information
4. Up-to-date Certificate of Immunization on BLUE FLORIDA HRS 680 form.
5. Custody verification if student is living with ONE parent (custody, divorce, court documents).
6. Notarized letter and proof of residence from family you reside with and address verification affidavit (Completed with Registrar).
7. Student’s social security card.

GRADES 1–5 REGISTRATION BEGINS AFTER THE LAST DAY OF SCHOOL

REQUIREMENTS FOR GRADES 1–5 REGISTRATION

IN COUNTY TRANSFERS :

1. Written proof of residence:  a signed lease, FPL bill, or water bill which includes YOUR NAME AND ADDRESS.
2. Custody verification if student is living with ONE parent (custody, divorce, court documents).
3. Notarized letter and proof of residence from family you reside with and address verification affidavit (Completed with Registrar).

OUT  OF COUNTY PUBLIC OR  PRIVATE SCHOOL  TRANSFERS:

1. Withdrawal form.
2. Name, address, phone, and fax number of last school attended.
3. Report card from previous school.
4. Official birth certificate or passport.
5. Written proof of residence:  a signed lease, FPL bill, or water bill which includes YOUR NAME AND ADDRESS.
6. Health physical exam ***See explanation on Registration Information.
7. Up-to-date Certificate of Immunization on BLUE FLORIDA HRS 680 form.
8. IEP testing and exceptional student education information.
9. Custody verification if student is living with ONE parent (custody, divorce, court documents).
10. Notarized letter and proof of residence from family you reside with and address verification affidavit (Completed with Registrar).
11. Student’s social security card.

PRESCHOOL:  Student must be 4 years old on or before September 1.
