In order for a student to participate in a sport at HCMS for the current school year, Sarasota County requires the following forms on file:
1. Physical Evaluation
2. Consent and Release from Liability Certificate
3. Insurance form

These forms are available in the front office. To be eligible to participate, a student must be in 7th or 8th grade and have a 2.0 grade point average or above from the previous quarter.
